Overview
- Provide in-home rehabilitation services to patients, focusing on improving functional outcomes and compliance with physician orders and regulatory standards.
- Conduct comprehensive patient evaluations, developing individualized treatment plans with measurable short- and long-term goals.
- Deliver skilled physical therapy services as per physician plan of care, ensuring quality and effectiveness.
- Collaborate with interdisciplinary team members to coordinate patient care and optimize treatment outcomes.
- Educate patients and caregivers on home exercise programs and care plans to promote recovery and independence.
- Participate in discharge planning, providing recommendations for continued care needs and follow-up services.
- Maintain accurate and timely documentation, including evaluations, progress notes, and discharge summaries.
- Supervise Physical Therapist Assistants (PTAs), ensuring compliance with oversight and documentation requirements.
- Adhere to productivity standards and manage scheduling requirements effectively.
Key Responsibilities & Duties
- Conduct timely, holistic patient evaluations in the home setting, identifying specific rehabilitation needs.
- Develop and implement individualized treatment plans with measurable goals tailored to patient conditions.
- Deliver skilled physical therapy services, ensuring alignment with prescribed frequency and duration of care.
- Monitor patient progress, adjusting care plans to support achievement of therapeutic goals.
- Collaborate with physicians and interdisciplinary teams to ensure coordinated and effective patient care.
- Educate patients and caregivers on home exercise programs and strategies for maintaining progress.
- Supervise and provide oversight to Physical Therapist Assistants (PTAs), ensuring compliance with care standards.
- Participate in discharge planning, offering recommendations for continued care and follow-up services.
- Maintain accurate documentation of evaluations, progress notes, and reassessments in compliance with standards.
Job Requirements
- Bachelor’s Degree in Physical Therapy; Doctorate in Physical Therapy (DPT) preferred.
- Active Physical Therapist License in New Jersey and AHA Basic Life Support (BLS) Certification.
- Minimum of 6 months of Physical Therapy experience; 3 years preferred.
- Proficiency in computer systems and electronic medical records (EMR).
- Ability to lift, push, and pull up to 50 lbs and perform physical work for extended periods.
- Strong organizational skills to manage scheduling and productivity standards effectively.
- Excellent communication skills to educate patients and collaborate with interdisciplinary teams.
- Commitment to delivering high-quality care and achieving positive patient outcomes.
- Ability to work independently in a home care setting while adhering to organizational policies.
