Overview
- Provide professional rehabilitation services to patients in a home care setting, ensuring high-quality, patient-centered care.
- Evaluate patients comprehensively to develop individualized care plans tailored to their specific needs and goals.
- Implement goal-oriented therapy programs to improve functional outcomes and enhance patient quality of life.
- Collaborate with interdisciplinary teams to ensure coordinated and effective care delivery.
- Supervise Physical Therapist Assistants (PTAs) and provide performance assessments and guidance.
- Maintain accurate and compliant documentation, including progress notes and discharge summaries.
- Monitor patient progress and adjust treatment plans to align with evolving needs and goals.
- Support discharge planning and recommend continued care or services as necessary.
Key Responsibilities & Duties
- Conduct patient evaluations within required timeframes, ensuring a holistic assessment of their condition.
- Develop and implement individualized treatment plans with measurable short- and long-term outcomes.
- Provide skilled physical therapy services in accordance with physician orders and established care plans.
- Collaborate with healthcare professionals to coordinate comprehensive care and services.
- Supervise and assess the performance of Physical Therapist Assistants (PTAs).
- Ensure compliance with documentation standards, including progress notes and reassessments.
- Adjust treatment plans based on patient progress and feedback to optimize outcomes.
- Participate in discharge planning, providing recommendations for continued care or services.
Job Requirements
- Bachelor’s Degree in Physical Therapy and active NJ Physical Therapist License required.
- Minimum of 6 months of Physical Therapy experience; Doctorate in Physical Therapy preferred.
- AHA Basic Life Support (BLS) Certification and proficiency in EMR systems required.
- Strong clinical judgment, patient assessment, and communication skills essential.
- Ability to work independently in a home health setting and collaborate effectively with teams.
- Physical capability to lift, push, and pull up to 50 lbs and remain active for extended periods.
