Primary Care Provider Key Responsibilities & Duties
- Conduct patient screenings, examinations, diagnoses, and treatments adhering to evidence-based guidelines.
- Provide timely follow-ups on patient needs, test results, and referrals.
- Collaborate with healthcare professionals to ensure coordinated care within a team-based model.
- Document patient care promptly and accurately using electronic health records systems.
- Engage in professional development and continuing medical education to maintain expertise.
- Participate in quality improvement initiatives and case conferences.
- Provide primary care services, including HIV and trans care, to a diverse patient population.
- Make appropriate internal and external referrals to support patient health needs.
Primary Care Provider Job Requirements
- Doctor of Medicine (MD) or equivalent degree with valid New York State certification/licensure.
- Current NYS Registration and DEA Controlled Substances Registration Certificate.
- Board certified or eligible in Family Medicine, Internal Medicine, or Infectious Disease.
- Minimum of 2 years of primary care experience; 3 years preferred.
- Experience in community-based primary care and HIV/trans care is a plus.
- Strong collaboration skills and commitment to patient-centered care.
- Bilingual proficiency in Spanish/English is advantageous.
- Current CPR certification and ability to work effectively with diverse populations.
