Physical Therapist Key Responsibilities & Duties

• Conduct comprehensive in-home evaluations and develop customized treatment plans for patients.
• Administer therapy sessions focusing on mobility, balance, strength, and pain management.
• Collaborate with referring physicians, healthcare teams, and caregivers to ensure effective care.
• Maintain accurate and timely documentation across multiple EMR systems.
• Educate patients and caregivers to promote effective home management strategies.
• Coordinate patient schedules and appointment times, ensuring efficient service delivery.
• Perform evaluations, start-of-care assessments, and discharge planning as required.
• Adapt to diverse agency protocols and documentation requirements.

Physical Therapist Job Requirements

• Active Florida Physical Therapist License is required for this position.
• A minimum of one year of home health experience is preferred.
• Proficiency in multiple EMR systems, including Pariox, Kantime, Kinnser, and others.
• Strong organizational and documentation skills to manage patient records effectively.
• Valid driver’s license and reliable transportation are essential for home visits.
• Professional demeanor and commitment to delivering exceptional patient care.
• High School Diploma or GED is required for this role.
• Flexibility to work Saturdays and adapt to patient scheduling needs.
