Field Nurse Supervisor Overview

• The Field Nurse Supervisor oversees patient care and staff performance in homecare settings, ensuring compliance with regulations and quality standards.
• Conduct supervisory visits to assess patient needs, care plans, and staff competency.
• Split time between fieldwork and office-based duties, managing a diverse case load.
• Collaborate with intake, scheduling, and operations teams for seamless care transitions.
• Participate in on-call rotations, providing clinical support and earning additional stipends.
• Utilize electronic medical records for accurate and timely documentation.
• Support process improvement initiatives through chart audits and quality assurance activities.
• Ensure culturally sensitive and age-appropriate care for diverse patient populations.

Field Nurse Supervisor Key Responsibilities & Duties

• Conduct in-home supervisory visits per regulations and agency policies.
• Assess patient environments and care plan implementation during visits.
• Evaluate field staff performance and provide coaching and training.
• Identify clinical concerns and communicate findings to the care team.
• Document supervisory visits accurately in the electronic medical record system.
• Respond to clinical inquiries and incidents from staff, patients, and families.
• Collaborate on case starts, transitions, and care plan updates.
• Participate in on-call rotations, providing clinical support as needed.
• Contribute to quality improvement initiatives through audits and process reviews.

Field Nurse Supervisor Job Requirements

• Active New York RN license in good standing is required.
• Associate’s degree in Nursing required; BSN preferred.
• Minimum 2–3 years of nursing experience; home care and supervisory experience strongly preferred.
• Current CPR/BLS certification is mandatory.
• Proficiency in electronic medical records; HHA exchange knowledge preferred.
• Knowledge of UAS system is advantageous.
• Reliable transportation, valid NY driver’s license, and proof of auto insurance required.
• Strong communication, coaching, and assessment skills.
• Familiarity with NYSDOH home care regulations and supervisory requirements.
