Field RN Supervisor Overview

• The Field RN Supervisor oversees home care operations, ensuring high-quality patient care and effective staff supervision.
• Conduct field-based nursing visits to patients' homes, providing clinical oversight and support.
• Develop and manage individualized care plans tailored to patient needs and conditions.
• Collaborate with interdisciplinary teams to coordinate comprehensive patient care.
• Ensure compliance with regulatory standards and organizational policies in home care services.
• Provide education and guidance to caregivers, patients, and field staff for optimal care delivery.
• Monitor patient progress and address changes in condition promptly and effectively.

Field RN Supervisor Key Responsibilities & Duties

• Conduct comprehensive patient assessments and initiate care plans during home visits.
• Provide medication management and educate patients and caregivers on proper usage.
• Supervise and support home health aides and other field staff in delivering care.
• Coordinate care with physicians, caregivers, and interdisciplinary team members.
• Monitor patient conditions and communicate changes to appropriate stakeholders.
• Ensure accurate and timely clinical documentation in compliance with regulations.
• Assess home environments for safety and suitability for care delivery.
• Participate in staff meetings, clinical reviews, and improvement initiatives.
• Maintain adherence to agency policies and state/federal home care regulations.

Field RN Supervisor Job Requirements

• Bachelor of Science in Nursing (BSN) from an accredited program.
• Active New Jersey RN license in good standing.
• Minimum of 1 year of nursing experience; 3 years preferred.
• Previous home care or assisted living experience strongly preferred.
• Strong clinical assessment and medication management skills.
• Excellent communication, documentation, and organizational abilities.
• Current CPR certification and ability to travel within assigned territory.
• Ability to work independently in a field-based environment.
• Familiarity with state and federal home care regulations.
