Field RN Supervisor Overview
- The Field RN Supervisor oversees home care operations, ensuring high-quality patient care and effective staff supervision.
- Conduct field-based nursing visits to patients' homes, providing clinical oversight and support.
- Develop and manage individualized care plans tailored to patient needs and conditions.
- Collaborate with interdisciplinary teams to coordinate comprehensive patient care.
- Ensure compliance with regulatory standards and organizational policies in home care services.
- Provide education and guidance to caregivers, patients, and field staff for optimal care delivery.
- Monitor patient progress and address changes in condition promptly and effectively.

Field RN Supervisor Key Responsibilities & Duties
- Conduct comprehensive patient assessments and initiate care plans during home visits.
- Provide medication management and educate patients and caregivers on proper usage.
- Supervise and support home health aides and other field staff in delivering care.
- Coordinate care with physicians, caregivers, and interdisciplinary team members.
- Monitor patient conditions and communicate changes to appropriate stakeholders.
- Ensure accurate and timely clinical documentation in compliance with regulations.
- Assess home environments for safety and suitability for care delivery.
- Participate in staff meetings, clinical reviews, and improvement initiatives.
- Maintain adherence to agency policies and state/federal home care regulations.
Field RN Supervisor Job Requirements
- Bachelor of Science in Nursing (BSN) from an accredited program.
- Active New Jersey RN license in good standing.
- Minimum of 1 year of nursing experience; 3 years preferred.
- Previous home care or assisted living experience strongly preferred.
- Strong clinical assessment and medication management skills.
- Excellent communication, documentation, and organizational abilities.
- Current CPR certification and ability to travel within assigned territory.
- Ability to work independently in a field-based environment.
- Familiarity with state and federal home care regulations.
