Referral Coordinator Overview
- The Referral Coordinator is responsible for managing and coordinating patient referrals as ordered by healthcare providers.
- Ensure patients are aligned with primary care providers before initiating referral processes.
- Collaborate with Patient Care Coordinators to ensure seamless referral operations and patient satisfaction.
- Maintain structured data fields in electronic medical records for accurate referral tracking.
- Coordinate referral appointments with external specialists and communicate with patients and providers.
- Secure insurance authorizations and ensure proper documentation for patient appointments.
- Participate in daily huddles to update referral statuses for patients scheduled for visits.
- Provide coverage at alternate locations and work evenings or weekends as required.
- Adhere to organizational values and contribute to advocacy events when needed.
Referral Coordinator Key Responsibilities & Duties
- Coordinate all aspects of patient referrals, including scheduling and tracking outcomes.
- Ensure proper paperwork is provided to patients for appointments as per provider orders.
- Track referral appointment results and update medical records accordingly.
- Maintain and update specialty provider lists, including accepted payor sources.
