Behavioral Health Counselor Overview
- The Behavioral Health Counselor provides integrated care using brief, solution-focused interventions to support patients' mental health and wellbeing.
- Collaborates with medical and dental teams to address psychosocial stressors and improve patient adherence to treatment plans.
- Focuses on short-term interventions, referring patients to external providers for ongoing therapy as needed.
- Works within an innovative healthcare model serving diverse populations, including immigrants and individuals experiencing homelessness.
- Contributes to enhancing the Patient Centered Medical Home model for high-quality, accessible care.
- Utilizes evidence-based behavioral health assessment and treatment modalities in a primary care setting.
- Engages in telehealth and time-limited interventions to maximize patient outcomes.
Behavioral Health Counselor Key Responsibilities & Duties
- Collaborate with primary care teams to deliver comprehensive behavioral health services to patients.
- Perform patient evaluations, establish differential diagnoses, and develop intervention plans.
- Maintain confidentiality and document care provided in compliance with organizational standards.
- Provide consultations and act as a consultant for other healthcare professionals as needed.
- Conduct comprehensive assessments and refer patients for further care within the organization or community.
- Utilize teaching-learning theory to educate patients and families, influencing positive health outcomes.
- Identify systemic issues requiring evaluation and collaborate on performance improvement processes.
- Contribute to the development of educational programs addressing diverse population needs.
Behavioral Health Counselor Job Requirements
- Clinical licensure as LCSW or LPC-MHSP in Tennessee required; 6+ months experience required, 2+ years preferred.
- Proficiency in Spanish, Arabic, or Kurdish preferred; experience with trauma and cognitive behavioral therapy advantageous.
- Ability to work independently in clinical decision-making and communicate effectively with patients and teams.
- Familiarity with evidence-based behavioral health assessment and treatment modalities essential.
- Flexibility and motivation to work in a primary care setting; prior integrated healthcare experience preferred.
- Commitment to innovative delivery methods, including brief interventions and telehealth.
- High School Diploma/GED required; additional certifications in relevant therapies preferred.
