Care Coordination Specialist Overview
- The Care Coordination Specialist is a key member of the healthcare team, ensuring comprehensive medical case management services for all primary care patients.
- Collaborate with clinical and support staff to manage patient enrollments, referrals, and administrative processes within the EMR system.
- Support patients in navigating insurance programs, including ADAP applications and authorization processes.
- Document hospitalizations, schedule discharge appointments, and monitor client progress to enhance patient outcomes.
- Participate in morning huddles and medical staff meetings, contributing to quality improvement initiatives.
- Coordinate referrals, ensuring proper documentation, insurance authorizations, and communication of outcomes to providers.
- Oversee program enrollment and manage resource distribution, including gift card allocation and record-keeping.
- Advocate for patients affected by homelessness, substance use, and HIV/AIDS, supporting the organization’s mission.

Care Coordination Specialist Job Requirements
- High School Diploma or GED required; relevant certifications are a plus.
- Minimum of 2 years of experience in a medical or related field preferred.
- Proficiency in using Electronic Medical Record (EMR) systems for documentation and data entry.
- Strong organizational skills and ability to manage multiple tasks in a fast-paced environment.
- Excellent communication skills for collaborating with clinical teams and supporting patient care coordination.
- Knowledge of insurance programs and processes, including ADAP, is highly desirable.
