Job Description

Registered Nurse Overview

  • The Registered Nurse role focuses on diabetes care and education, supporting members with newly diagnosed diabetes and those with high A1Cs.
  • Collaborate with the Quality Assurance RN Manager to develop personalized diabetes management plans for members.
  • Provide education and support tailored to members' lifestyles, beliefs, and cultural backgrounds.
  • Enable members to understand blood glucose management and develop coping skills for daily challenges.
  • Assist members in identifying patterns for improvement in self-management practices.
  • Educate members on the use of prescribed devices, including insulin pens and glucose monitoring tools.
  • Support members in reducing risks of complications and emergency room visits through effective diabetes management.
  • Participate in additional duties and projects as assigned by management.

Registered Nurse Key Responsibilities & Duties

  • Collaborate with members and providers to create effective diabetes management plans.
  • Conduct telephonic consultations to address gaps in care and high A1Cs.
  • Provide education on healthier food choices, physical activity, and medication adherence.
  • Assist members in coping with lifestyle changes required by diabetes management.
  • Educate members on the use of diabetes management devices and tools.
  • Support members in reducing risks of complications and emergency visits.
  • Maintain effective communication with physicians and healthcare teams.
  • Participate in quality assurance and improvement processes.

Registered Nurse Job Requirements

  • Valid New York State Registered Nurse (RN) license required.
  • Bachelor’s degree in Nursing preferred; Diabetes Care and Education Specialist certification required.
  • Minimum three years of Diabetes Care and Education Specialist experience required.
  • Two years of nursing experience in ambulatory care, community health, or medical/surgical settings required.
  • Strong clinical assessment, patient education, and motivational interviewing skills needed.
  • Intermediate computer literacy and proficiency in Microsoft Outlook, Word, and Excel preferred.
  • Bilingual in Spanish and valid NYS Driver’s license preferred.
  • Flexibility to work evenings and weekends as required.
