- The Program Manager – Care Coordination oversees grant-funded programs within the Department of Support Services, ensuring compliance, quality, and operational excellence.
- Lead initiatives such as Care Coordination, Be Into Health, and PrEP/PEP Navigation to deliver impactful services to target populations.
- Collaborate with internal and external stakeholders to ensure seamless program execution and adherence to regulatory requirements.
- Drive quality assurance and improvement activities, enhancing service delivery and patient satisfaction.
- Recruit, train, and supervise program staff, fostering a culture of accountability and professional growth.
- Adapt strategies to meet evolving community needs and funder requirements, ensuring program sustainability.
- Represent the organization at various venues, promoting services and building partnerships.
- Contribute to organizational activities, including multidisciplinary meetings and events, to support the mission.
Program Manager – Care Coordination Key Responsibilities & Duties
- Ensure compliance with service levels, quality standards, budgets, and reporting requirements for assigned programs.

Program Manager – Care Coordination Job Requirements
- Bachelor’s degree in a relevant field with 5 years of full-time experience required.
- Minimum 2 years of supervisory experience managing paid staff.
- Proficiency with systems such as eCW, eSHARE, eICARE, Healthix, and PSYKE preferred.
- Strong organizational, verbal, and written communication skills with a focus on cultural competence.
- Experience working with individuals facing challenges such as mental illness, substance use disorders, or chronic illness.
- Proficiency in MS Office applications including Outlook, Word, Excel, and PowerPoint.
- Ability to adapt to changing community needs and funder requirements effectively.
- Willingness to travel to partner sites and work occasional evenings or weekends as needed.
