• The Case Manager – Medical provides direct case management to patients in a primary care clinic, ensuring access to and retention in medical care.
• Collaborates with multidisciplinary teams to support patients' health outcomes through proactive care coordination and service linkage.
• Develops individualized care plans based on comprehensive assessments and reassessments to address patient needs effectively.
• Coordinates access to specialty care, mental health services, and substance use treatment, ensuring seamless integration with primary care.
• Maintains accurate documentation in electronic and paper records to meet reporting requirements and support continuous quality improvement.
• Identifies and recruits individuals from diverse communities to facilitate early access to treatment and social services.
• Ensures compliance with health codes and organizational standards, including immunization and communicable disease testing.
• Supports organizational goals through active participation in clinic and client services activities.

Case Manager – Medical Key Responsibilities & Duties

• Conduct intake, assessments, and reassessments to develop individualized care plans for patients.
• Coordinate patient access to medical services, entitlements, and community resources.
• Obtain prior authorizations for pharmacy and medical services not covered by insurance plans.
• Maintain accurate and timely documentation in electronic medical records and agency databases.
• Collaborate with primary care providers and multidisciplinary teams to ensure positive health outcomes for patients.
• Recruit individuals from target communities to facilitate access to care and services.
• Participate in continuous quality improvement activities to enhance service delivery.
• Perform other duties as assigned to support organizational objectives.

Case Manager – Medical Job Requirements

• Bachelor’s degree in social science, psychology, social work, health, or equivalent experience required.
• Minimum of three years of relevant work experience in case management or related field preferred.
• Experience working with diverse communities, including individuals living with HIV/AIDS, immigrants, and LGBT populations.
• Proficiency in one or more Asian languages is preferred.
• Strong written and verbal communication skills in English required.
• Ability to manage multiple tasks effectively in a fast-paced environment.
• Proficiency in using common office software and electronic medical records systems.
• Compliance with immunization and communicable disease testing requirements.
